To ensure we can complete the process efficiently, there are a few key details and documents you’ll need to prepare ahead of the call.
If you don't have them all to hand, that is not an issue, we can still complete the bulk of the forms and fill in any missing information once you have confirmed it.
Please have the following information ready:
1. Your Personal Information
- Full legal name
- Date of birth
- Current address
2. Details for Your Attorney(s)
- Full legal names of the people you wish to appoint as your attorneys (those who will make decisions on your behalf)
- Their date of birth and current address
- Their relationship to you (e.g. spouse, friend or sibling)
- Their contact details (phone number and email)
3. Replacement Attorney(s) (Optional)
- If you’d like to appoint replacement attorneys (who can step in if your primary attorneys are unable to act), please have the same details as above ready for them.
4. Certificate Provider
- If you have chosen a Certificate Provider (the person who will confirm you have the mental capacity to make an LPA), please have their full name, address, and occupation ready.
5. Preferences for Decision Making
- Do you want your attorneys to act jointly (together) or jointly and severally (independently)?
- Do you have any specific instructions or preferences for how your attorneys should make decisions?
6. Financial and Property Decisions LPA (if applicable)
- Bank names or specific information about assets you’d like your attorneys to manage (if needed).
7. Health and Welfare LPA (if applicable)
- Any specific preferences for medical care, living arrangements, or end-of-life care that you would like to include.
8. Witness for Your Signature
- Your LPA document will need to be signed in the presence of an independent witness. While this can be arranged later, it may be helpful to think about who could act as your witness.
